|Title:||IDD Intake & Continuity of Care Coordinator|
General Description: This position is responsible for coordination and provision of direct care service in facilitating enrollments, transfers, and discharges for persons with Intellectual and Developmental Disabilities into and out of community programs, Medicaid waiver programs, ICF-MR, and State Supported Living Center services. Additional duties include maintaining the IDD waiting list, permanency planning for persons under age 22 in institutions and ICF-MR’s, and managing special projects for Intellectual and Developmental Disability services as needed. Work involves developing and maintaining both short and long term contact with clients, clients’ families, service providers, and community agencies. The position represents the Center as a liaison, and informs agencies, consumers, and families of services and supports. This positions requires the ability to work well with a team of staff, however much of the work is completed independently and with significant latitude for the use of initiative and independent judgment. Professionalism is mandatory.
Qualifications: (Education & Experience):
- Requires a Bachelor’s Degree in a social services related field or meet credentialing requirements for IDD Service Coordination.
- Prefer a minimum of 3 years experience in IDD services.
- Must be familiar and competent in using standard office software (Excel, Word, etc…)
- Must have a current Texas Driver’s License and qualify for Center insurance coverage.
